Multi-Specialty Clinic

Transition Safely from Hospital to Home

For patients, the transition from hospital stay to home is a critical time in their healing cycle – especially for those who are recovering from complex and chronic conditions. To help smooth this process and bridge the gap, Sherman Oaks Hospital offers a Multi-Specialty Clinic on its medical campus. Now, your hospital provider can extend the continuity of care between your hospital stay and your follow-up Primary Care appointment by arranging a clinic visit within the first seven days of discharge to continue to assess your ongoing medical needs throughout your recuperation.

Did you know? Almost 60% of hospitalized Medicare patients did not have contact with a physician within two weeks following discharge. For patients with ongoing medical needs, misconnections are major contributors to hospital readmissions.1

We are here to address your concerns and help you get back on your feet!
