the gap, Sherman Oaks Hospital offers a Multi-Specialty Clinic on its
medical campus. Now, your hospital provider can extend the continuity
of care between your hospital stay and your follow-up Primary Care appointment
by arranging a clinic visit within the first seven days of discharge to
continue to assess your ongoing medical needs throughout your recuperation.
Did you know? Almost 60% of hospitalized Medicare patients did not have contact with
a physician within two weeks following discharge. For patients with ongoing
medical needs, misconnections are major contributors to hospital readmissions.1

How we can help
The Multi-Specialty Clinic offers services from a team of experts, which
include physicians, nurses, social workers, and pharmacists. Our services include:
- Diagnosis / Symptom management
- Health education and coaching
- Medication review and optimization
Care is provided by
Epic Medical Group. These physicians will provide a seamless transition for patients while
managing their conditions at the clinic, returning patients to their primary
care physician once they are stabilized. They will provide exceptionl
care throughout each patient’s journey – from hospitalization
